          For the First Conviction of DUI: minimum of 180 days of license revocation and up to a maximum of one (1) year of license revocation.

          For a Second Conviction of DUI Within Five (5) Years of Another DUI Conviction: minimum of five (5) years of license revocation. You may be eligible for a hardship reinstatement of a driver’s license after one (1) year.

          For a Second Conviction of DUI Outside of Five (5) Years After Another DUI Conviction: minimum of 180 days of license revocation and up to a maximum of one (1) year of license revocation.

          For a Third Conviction of DUI Within Ten (10) Years of Another DUI Conviction: minimum of ten (10) years of license revocation. You may be eligible for a hardship reinstatement of a driver’s license after two (2) years.

          For a Third Conviction of DUI Outside of Ten (10) Years After Another DUI Conviction: minimum of 180 days of license revocation and up to a maximum of one (1) year of license revocation.

          For a Fourth Conviction of DUI, Regardless of When Prior Convictions Occurred: mandatory permanent revocation. No hardship reinstatements permitted.

          For a DUI Manslaughter Conviction: mandatory permanent revocation. If there are no prior DUI-related convictions, you may be eligible for a hardship reinstatement of a driver’s license after five (5) years.

          For a Conviction of Vehicular Manslaughter, DUI Serious Bodily Injury, or Vehicular Homicide: minimum of three (3) years of license revocation.
